INSTRUCTIONS

Previous applicants please note: You must apply anew with all information and documentation whether or not you received an award. No previously submitted files will be accepted by the Committee.

Because we may be considering you for an interview, it is essential that you provide the most current and accurate contact information. If this information changes, please email ccbscholarships@gmail.com.

You must complete and submit your application online by Friday, May 15, 2020. All additional documentation must be received via email to ccbscholarships@gmail.com no later than Friday, May 15, 2020. The documents include the application, letters of recommendation, doctor’s statement of legal blindness, and personal statement.

Required Documentation:

In order to expedite your application, please send the following:
A. have official transcripts emailed directly from issuing institutions to the CCB Scholarship Committee as soon as transcripts become available.

B. Please provide a doctor’s statement or a statement from a qualified professional declaring that you are legally blind with a diagnosis and your visual acuity in both eyes.

C. Please provide two letters of recommendation from instructors, counselors, employers or others who can speak to your qualifications.

D. Please provide a 500-to-600-word personal statement giving your purpose in undertaking college work and your vocational goals. Give specific information to help us understand how you got to where you are and what you intend to achieve upon graduation.

All documentation must be received no later than Friday, May 15, 2020. All official transcripts must be received no later than Wednesday, July 15, 2020 by email to ccbscholarships@gmail.com. Please note in the subject line name of applicant and item being submitted, e.g. John Smith Letter of Recommendation. Acceptable file types for submitted documents are Microsoft Word documents, (.docx), or accessible PDF.
